Before Your Visit

We want all of our patients to feel completely prepared when they visit our dental office. After all, we consider ourselves your partners in creating and maintaining a beautiful, long-lasting smile, and to do that it’s important for you to feel well-informed and involved every step of the way! Your First Visit

If you haven’t already completed your patient forms, you’ll need to do so at check-in. Once we’ve finished processing the paperwork, we’ll call you back to the treatment area to begin your appointment. We’ll take any necessary X-rays and carefully examine your teeth and gums for warning signs of decay and infection. Finally, we’ll discuss what kind of dental treatment you’ll need in the near future and help you schedule your next visit appropriately.

Patient Forms

The forms that you fill out near the beginning of your first visit tell us about your dental history and the state of your current health; this tells us what we need to keep an eye out for during your examination.
